The rail fastenings fractured when they were used in a humid tunnel about two years. The fracture reasons were analysed with means of macroscopical observation,scanning electronic microscope and X ray energy spectrometer,metallography microscope and so on. The result shows that environment is stronger acid and the fastenings are seriously rusty,a small quantity of Cl and S element is detected in the corrosion. The fracture origin lies in the symmetric inner arc where the tensile stress is ultimate and the narrow gap happened because of serious corrosion. The shell pattern is apparent ,so the fastenings failure belongs to corrosion fatigue cracking. It can be prevented by using better corrosion resistance steel,increasing tempering temperature properly,hot galvanizing and reducing the fastenings serviceable life.

In order to test the correctness of barcode,the system uses PC and reader of barcode to detect and compare the data of barcode,and uses man-machine interface which is developed by VB software to display the barcode value and set the appropriate parameters,then records and keeps data. Meanwhile the results are sent to Freescale single chip microcomputer through the serial port,then the signal is transferred to the nRF24L01 wireless module sender through SPI bus. At last,the signal is sent into the single chip microcomputer by the nRF24L01 wireless module through SPI bus,then controls the operation of stepper motor according to the correctness of barcode and output the barcode. The system is easy to operate,can be used for teaching and product quality detection.

Objective
To explore the clinical application effect of the Herbert screw for the treatment of medial malleolus fracture.
Methods
Selects 38 cases of Herbert screw for the treatment of medial malleolus fracture from July 2011 to August 2013 in orthopedics department in the affiliated hospital of Jianghan University,patients were followed up for 3-6 months,according to the American Society for Ankle Surgery(AOFAS)score standard for curative effect evaluation.
Results
Finally 36 patients completed the study,2 cases missed,there were 25 excellent,8 well,3 satisfaction,good rate 91. 67% .
Conclusion
Herbert screw for the treatment of medial malleolus fracture has achieved good effect,for the treatment of medial malleolus fracture,it provides a new choice and it is worthy for popularizing.

Through the laboratory examination results of a case with HbH complicated with G6PD deficiency in acute hemolysis,to analysis the interaction of hematology parameters when the two diseases exist at the same time and complicated with acute hemolysis,and investigates the meaning to diagnosis and therapy. Through this case,obtains the conclusions as below:(1) In hemolytic patient,with MCV normal or higher,it can not exclude the possibility of thalassemia,and need to have further examination to confirm;(2)In acute hemolysis,with G6PD normal,it can not exclude the possibility of G6PD deficiency,need to reexam G6PD 1 month after acute hemolysis become normal,to obtain the real result. The clinical diagnosis and therapy can not believe the temporary results of laboratory fanatically,and should consider comprehensively,so cautiously treats the hemolysis and avoids becoming worse. Because that the both two are genetic disease,the most effective way to prevent and treat the disease are antemarital physical examination and prenatal examination so as to avoid the birth of infant with the defect.

Objective
To study the clinical effect of Sulodexide on albuminuria in early diabetic nephropathy.
Methods
65 cases of patients with early diabetic nephropathy were randomly divided to a control group including 30 cases and a treatment group including 35 cases. Both groups have the treatment 8 weeks. The control group orally take Losartan 50 mg/d for 4 weeks. The treatment group on the basis of normal treatment(Losartan potassium 50 mg,one time/d),adds Sulodexide 600 LSU,qd,iv ,one time/d,after 2 weeks,changes to use Sulodexide tablet 200 mg/d for 6 weeks (totally 8 weeks).
Results
After treatment,B2-MG,BUN,CREA and 24-hour urinary protein is significantly lower than that before treatment for both groups(P<0. 05 or P<0. 01). These indexes of treatment group are better than those of control group.
Conclusion
Sulodexide can relieve albuminuria effectively in early diabetic nephropathy.

Objective
To observe the effects of dexmedetomidine to anesthesia laparoscopic surgery on postoperative recovery phase CO2 accumulation in patients.
Methods
Chose patients undergoing laparoscopic operation. Before the end of operation about 40 min measured arterial blood gas. Selecting 40 cases of patients with PaCO2 70-90mmHg and pH<7. 30. They were randomly divided into a control group(group N)and dexmedetomidine group(group D) with 20 cases in each. Before the end of operation about 30 min. N group were treated with 0. 9% sodium chloride 10 mL slow intravenous injection. D group were given the dexmedetomidine dubbed 4 μg/mL slow intravenous with dose concentration 0.8 μg/kg. Recorded two groups of patients at extubation(T0),5 min after extubation(T1),30 min after extubation(T2)of heart rate,blood pressure and Riker score. Two groups of patients from extubation to can returned to the ward satisfaction time(T3).
Results
Compared the group D with N,at T0,T1,T2 time,heart rate,blood pressure,and Riker scores were statistically significant(P <0. 05). Comparison of the two groups was not statistically significant from extubation begin to restore satisfactory and could return to ward(P> 0. 05).
Conclusion
Laparoscopic surgery,CO2 accumulation in patients with hypercapnia recovery phase of anesthesia,given dexmedetomidine is safe and effective.
