动脉全血和静脉血浆中钾和钠比较分析

Arterial Blood and Venous Plasma Comparative Analysis of Potassium and Sodium

摘要: 目的利用血气分析仪和电解质分析仪分别测定动脉全血和静脉血浆中的钾离子和钠离子浓度, 比较两者之间的差异和推导两者的线性关系, 探讨其临床意义.方法用美国生产的GEM PREMIER 3000血气分析仪和Roche公司生产的ROCHE 9180型电解质分析仪分别检测85例患者动脉全血和静脉血浆的钾离子和钠离子浓度,比较动脉全血和静脉血浆的钾离子、 钠离子之间的差异和相关性.结果动脉全血和静脉血浆的钾离子浓度差异有统计学意义(P<0.05), 动脉全血、 动脉血浆和静脉血浆的钠离子浓度差异无统计学意义(P>0.05),动静脉血浆的钾离子或钠离子差异无统计学意义(P>0.05); 并且动脉全血和静脉血浆中钾离子浓度存在着一定的线性关系.结论动脉全血中钾离子浓度不能等同于静脉血浆中的钾离子浓度, 静脉血浆中钾离子的参考范围不能套用在动脉血气分析中, 根据笔者推导的直线回归方程静脉血浆钾离子(A)=1.445× 动脉全血钾离子(V)-1.273(mmol/L), 在知道动脉全血血气分析中钾离子浓度时, 可以大致推算出静脉血浆钾离子浓度, 临床医生可以比较全面地判断患者有无钾离子异常.

Abstract: ObjectiveArterial blood gas analyser and electrolyte analyser were used to measure arterial blood and venous plasma potassium and sodium ion concentration, the experimental data were statistically compared the differences between them and derived the linear relationship, discussed the clinical significance.Methods Using American production of GEM PREMIER 3000 blood gas analyser and Roche production of ROCHE 9180 electrolyte analyser, detected in 85 cases with arterial blood and venous plasma the potassium and sodium concentration, compared arterial blood and venous plasma potassium, sodium and relevance.Results Arterial blood and venous plasma potassium concentration were significantly different(P<0.05), arterial blood, arterial plasma and venous plasma sodium concentration were not significant differences(P>0.05),arterial plasma and venous plasma potassium or sodium concentration were not significantly different(P>0.05); and arterial blood and venous plasma potassium concentration in a certain linear relationship existed.Conclusionarterial blood potassium concentration is not the same with venous plasma potassium concentration, but according to the experimental data, can roughly calculate the linear regression equation of venous plasma and arterial blood potassium ion concentrationvenous plasma potassium ion(A) =1.445×artery blood potassium ion(V)-1.273(mmol/L), so when the doctor know the potassium concentration in arterial blood,can roughly calculate the potassium concentration in venous plasma, the doctor can comprehensively judge the potassium situation in patient.
