Analysis of Association Between Marital Status and Prognosis of Liver Cancer Patients Based on SEER Database

摘要: 目 的 研 究 婚 姻 状 况 与 肝 癌 患 者 预 后 的 关 系 。 方 法 从 美 国 SEER 数 据 库 共 提 取 9 759 名患者数据,采用 Log-rank 单因素和多因素 Cox 风险比例模型评估婚姻状况与肝癌患者 预后的关系。结 果 离婚或分居患者(MHR = 1. 24,95%CI(1. 14,1. 34),P < 0. 001)、单身患 者(MHR = 1. 22,95%CI(1. 13,1. 32),P < 0. 001)、寡 居 患 者(MHR = 1. 21,95%CI(1. 09, 1. 33),P < 0. 001)比已婚患者的死亡风险都高,此外已婚患者的生存率高于离婚或分居、单身、 寡居患者。在不同年龄、人种、肿瘤分期和肿瘤分化程度的组别下肝癌患者的婚姻状况影响有所 差 异 ,中 年 组(χ2 = 99. 6,P < 0. 001)、白 种 人(χ2 = 84. 6,P < 0. 001)、肿 瘤 Ⅰ 期(χ2 = 87. 7, P < 0. 001)、高分化(χ2 = 67. 1,P < 0. 001)和中分化组(χ2 = 65. 0,P < 0. 001)内婚姻状况的 影响差异具有统计学意义。结 论 婚姻状况在一定程度上可以作为肝癌患者的独立预后因素, 且已婚肝癌患者的死亡风险系数低于其他婚姻状态患者。

Abstract: Objective To investigate the influence of marital status on the prognosis of patients with liver cancer. Methods Information from 9 759 patients was extracted from the SEER database,and univariate Log-rank and multivariate Cox risk scale models were used to evaluate the relationship between marital status and the prognosis of liver cancer. Results The death risk of divorced or separated patients(MHR = 1. 24,95%CI(1. 14,1. 34), P < 0. 001),single patients (MHR = 1. 22,95%CI(1. 13,1. 32),P < 0. 001), widowed patients(MHR = 1. 21,95%CI(1. 09,1. 33),P < 0. 001)was higher than that of married patients,and the survival rate of married patients was higher than that of divorced,separated,single,and widowed patients. There were significant differences in the effects of marital status among HCC patients with different ages,races,tumor stages,and degrees of tumor differentiation,and there were statistically significant differences in the effects of marital status among the middle-aged group(χ2 = 99. 6,P < 0. 001),White group(χ2 = 84. 6,P < 0. 001),tumor stage I group(χ2 = 87. 7,P < 0. 001),highly differentiated group(χ2 = 67. 1,P < 0. 001)and moderately differentiated group(χ2 = 65. 0,P < 0. 001). Conclusion Marital status can be an independent prognostic factor for patients with liver cancer to a certain extent,and the death risk coefficient of married patients with liver cancer is lower than that of patients with other marital statuses.
