核心训练对治疗青少年特发性脊柱侧凸效果的Meta分析

A Meta-analysis of Core Training Effect on Treating Adolescent Idiopathic Scoliosis

摘要: 目 的 :目 的 运用Meta分析系统评价核心稳定性训练对青少年脊柱侧凸(AIS)患者的改善效 果。方 法系统检索知网、万方、维普、PubMed、WebofScience五大数据库,检索方式采用主题 词+自由词的方式,检索时间截止至2023年7月31日。采用Revman5.4和Stata17.0进行Meta 分析,对结局指标(Cobb角、POTSI、SRS-22)进行合并效应量、亚组分析,对性别差异进行亚组 分析。结 果 共纳入14篇文献,包括755名研究对象。Meta分析显示:与对照组相比:①核心 训练能显著改善Cobb角(SMD =-1.42,95%CI(-1.91,-0.93),P < 0.05)、减小POTSI指 数(SMD =-1.11,95%CI(-1.70,-0.52))和提高 SRS-22(SMD = 2.01,95%CI(1.62, 2. 41)),差异有统计学意义;② 亚组分析结果显示:核心肌力训练(SMD =-2.07,95%CI (-2.84,-1.30))减小 Cobb角的效果优于核心稳定性训练(SMD =-0.84,95%CI(-1.28,-0.40));干预周期中,8周的干预时间最佳(SMD =-1.92,95%CI(-2.42,-1.41));干预 频率中,1次/d的频率最佳(SMD =-1.48,95%CI(-1.99,-0.98));在性别分析中,男女混 合组(SMD =-1.48,95%CI(-2.14,-0.82))的治疗效果要优于全女生组(SMD =-1.42, 95%CI(-1.87,-0.96))。结 论 核心训练对青少年特发性脊柱侧凸具有明显的改善效果,可 有效减小AIS患者的Cobb角,其中核心肌力训练对减小Cobb角有更明显的效果,更合适的干预 周期为8周,干预频率为1次/d效果最佳,男女混合组的治疗效果优于全女生组,核心训练也可 改善SRS-22,降低POTSI。

Abstract: ObjectiveTo evaluate the improvement effect of core stability training on adolescent idiopathic scoliosis (AIS) patients using Meta-analysis. Methods The systematic search of five major databases,CNKI,Wanfang,VIP,PubMed,and Web of Science,was performed by subject words + free words,and the search time was up to July 31,2023. Meta-analysis was performed using Revman 5.4 and Stata 17.0. Combined effect sizes and subgroup analyses were performed for the outcome indicators(Cobb angle, POTSI,and SRS-22),and subgroup analysis was performed for gender differences.ResultsA total of 14 articles were included,involving 755 research subjects. The results of the Meta-analysis showed that compared with the control group, ① core training significantly improved the Cobb angle(SMD =-1. 42,95%CI(-1. 91,-0.93),P < 0. 05),reduced the POTSI index(SMD =-1.11,95%CI(-1.70,-0.52)),increased the SRS-22(SMD = 2.01,95%CI(1.62,2.41)),and the difference was statistically significant; ② the results of subgroup analyses showed that core plyometrics training (SMD =-2.07,95%CI(-2.84,-1.30))was more effective in reducing Cobb′s angle than core stability training (SMD =-0. 84,95%CI(-1.28,0.40));among the intervention cycles,8 weeks was the best intervention duration(SMD =-1.92,95%CI (-2.42,-1.41)),among the intervention frequencies,1 session/day was the best frequency(SMD =-1. 48,95%CI(-1.99,-0.98)),and in the gender analysis,the mixed group(SMD =-1.48,95%CI(-2.14,-0.82))had a better treatment effect than the all-girls group(SMD =-1.42,95%CI(-1.87,-0.96)) Conclusion Core training has a significant improvement effect on AIS and can effectively reduce the Cobb angle in patients with AIS. Core plyometric training has a more obvious effect on reducing the Cobb angle,and a more appropriate intervention cycle of 8 weeks,the intervention frequency of 1 time/day has the best effect. The treatment effect of the mixed group of men and women is better than that of the all-girls group,and the core training also improves the SRS-22 and reduces the POTSI.
